5 Phases of completion  

TIME FRAMES FOR PROGRESS THROUGH THE TREATMENT PROGRAM ARE ENTIRELY DEPENDENT UPON YOUR MOTIVATION, EFFORT, CHOICES, AND BEHAVIORS.

ORIENTATION

  • Know staff and resident’s names
  • Pass Resident Handbook Test
  • Take Casey Life Skills Assessment
  • Complete SASSI assessment
  • Demonstrate required personal hygiene
  • Talk treatment with cottage staff

ACCEPTANCE

  • Participate in individualized Treatment Plan meeting and help form treatment goals
  • Complete Line Staff Goal*
  • Begin working on Life Skills Packet
  • Begin working on Anger Management Workbook
  • Begin Substance Abuse Treatment Program (Seeking Safety-if applicable)
  • Complete a life story and discuss this with counselor
  • Actively participate in all group and individual therapy sessions

LEARNING

  • Complete Line Staff Goal*
  • Consistently demonstrate appropriate responses to authority
  • Actively work on my therapy issues, both in group and individual sessions
  • Be a positive role model for lower phased residents
  • Show progress on Life Skills Packet
  • Show progress on Anger Management Workbook (if applicable)
  • Read a biography about a famous woman and write a one-page summary.
  • Can take GED (If age appropriate)

TRUST

  • Complete Casey Life Skills packet
  • Complete Anger Management Workbook
  • Able to perform KP duties with no redirection
  • Complete Line Staff Goal*
  • Provide positive leadership to lower phase residents
  • List 5 career interests and gather information on at least one of them to discuss with staff
  • Correctly fill out at least one job application
  • Complete all the phases of the Substance Abuse Program
  • Actively participate in all group and individual sessions


​Relapse prevention

  • Complete Relapse Prevention Plan
  • Practice high risk situation coping scenarios
  • Serve as a mentor to resident(s) in orientation phase
  • Complete mock job interview with designated staff
  • Establish a community support system and discuss this with counselor
  • Apply for employment in the community in which you live (if applicable)If planning to attend college, complete necessary paperwork
  • Successfully complete an Off Campus Assignment**with staff
  • Demonstrate the ability to function independently on a consistent basis (Ex. daily living skills)
  • Actively participate in all group and individual sessions.
Also, we at Otter Creek recognize the importance of putting skills into practice prior to re-entry to the community. As our residents approach the fifth, and final,  phase of our program they are moved to our transitional living unit on-campus and placed on the caseload of our Transitional Living Coordinator. Within the unit, the resident’s treatment focus shifts from previously completed mental health treatment goals and objectives, to increasing independent living skills and preparing for a successful transition back to the community. Youth in this group are exposed to opportunities to effectively manage an increase in freedoms and responsibilities, as well as managing finances and employment skills. In order to better prepare for transition, these residents are also eligible for increased home visits when appropriate.
